-JFSA has a full-time opportunity for a Case Manager in the Behavioral Health unit within Ascentia, our program for people with chronic and severe mental health disabilities. -The Case Manager will institute a stabilization and recovery process that encourages independence and reduces crises, provide practical information, strategies and skills that will achieve recovery goals and monitor service provided by other staff/agencies in assisting clients toward their goal of independence. JFSA Care at Home, a premier Home Health agency, is looking for a knowledgeable and energetic Coding and Billing/Scheduling Specialist. This fast-paced position will be responsible for the coding of the Medicare/Medicaid assessments, creation of 485's, reviewing OASIS items to determine consistency with other clinical documentation and diagnoses to be coded for the billing process. The medical coder?s basic responsibility is to accurately translate patient information into alpha-numeric medical code. Information handled by medical coders include patient treatment, health history, diagnosis, and related information. The medical coder should be detail-oriented as possible as the dispersion of proper payment and creation of precise, easily processed medical bills depend on their coding expertise. Other responsibilities include: insuring that all visit information is correctly entered into data management systems to generate a claim for billing, monitoring claim status and providing appropriate follow up to ensure the timely payment of claims. In addition to Billing/Coding responsibilities, this position is responsible for scheduling all licensed disciplines/clinicians to provide Skilled Nursing services. The Employment Program focuses on assisting consumers in securing and maintaining employment as well as encouraging consumers to be knowledgeable of their opportunities for career advancement. -Under the supervision of the Employment Manager, the Employment Specialist provides group and/or individualized job placement assistance to adults with severe mental illness or other disabilities. - The Employment Specialist will also conduct specialized outreach to individuals with disabilities and other challenges in the Mental Health and Jewish communities. - This includes assessing their skills as they pertain to the local job market and assisting them in setting and reaching appropriate employment objectives. JFSA Care at Home is seeking Housekeepers/Maids to clean the personal residences of our elderly clients. -The right applicant for this position will have the ability to assess and diagnose, develop treatment plans, work closely with agency's psychiatrists, facilitate therapeutic and supportive groups, maintain communication with clients and family members as well as maintain billing and medical records. -JFSA has a part-time (24-32 hrs week) opportunity for a Licensed Social Worker/Therapist in our Ascentia Behavioral Health unit to work with a diverse population of adults with mental illness and to provide counseling services to individuals, couples, families and groups.

JFSA Care at Home is actively seeking an experienced and customer service oriented RN Clinical Intake Coordinator to manage the intake of new clients for private duty nursing, skilled nursing and social support services. The Clinical Intake Coordinator with work collaboratively with JFSA Care at Home staff to ensure effective and efficient coordination of care. Responsibilities will include telephone intake for various services, evaluation and assessment of clinical symptoms, authorization of treatment plans and coordination/scheduling of follow up services. JFSA is seeking on-call Secondary Community Support Professional to provide supportive services to adults with severe mental disabilities. This position is responsible for working with agency clients regarding the nature of severe mental illness and the potential usefulness of medication, case management, activity groups and other services. They will instruct clients on activities of daily living, including shopping, budgeting, traveling, and community adjustment. They can provide transportation to services and resources and assist in monitoring client?s mental status and general functioning. |
